Chapter 267: A Crowd Shows Respect

~8 min read 1,410 words

"Wu Zong, I'll go take a look with you," Ji Xiang said, turning his head.

"I doubt there's anything for us to do," Wu Zong smiled. "When a trauma patient is opened up and they see a Retroperitoneal hematoma, they sometimes call us to take a look. You can't open it; it's more trouble if you do. The pressure in a Retroperitoneal hematoma is quite high, and it can stop minor vascular bleeding on its own."

Ji Xiang stood up and followed behind Wu Zong. Wu Zong didn't hold back, sharing all his Clinical experience with Ji Xiang.

"Our department doesn't get many emergencies. What I hate most is when nurses from other departments encounter a patient with difficulty urinating and don't even try, just calling us as soon as an elderly male needs a catheter. It's a bad habit they've been spoiled with. Whenever I run into this, I usually give their Resident Chief a scolding."

"Same on my end, it seems to be a common problem," Zhao Zhe caught up and opened the floodgates. "When I'm on duty, I basically have to go to other departments to insert catheters every shift. There's really no helping it; they don't even know the most basic operations. Sometimes I want to smash a book on basic medical skills into the faces of the doctors in other departments."

"But then I think, I don't know how to do Closed Thoracic Drainage or lumbar punctures either, so I'd better..."

Wu Zong glared at Zhao Zhe. Whose side was he on?

Zhao Zhe chuckled and didn't continue. Hospital affairs were a tangled mess; who could ever make sense of them?

Besides, he had a gentle temper and was long used to being bossed around. Wu Zong had only glared at him and hadn't said anything harsh, which to Zhao Zhe was already a stroke of great luck.

Seeing the three of them heading out, Lu Gang hesitated for a moment, but still followed.

"Lu Zhuren, aren't you going home?" Wu Zong asked.

"Going to take a look," Lu Gang said in a muffled voice.

It was obvious he was carrying some emotions.

"Geez, what's there to see at a Urology Department intraoperative consultation?" Wu Zong said.

However, he didn't drive Lu Gang away. If he wanted to watch, let him watch. They were all Clinical veterans; taking a look wouldn't cost them a piece of flesh.

Arriving at the operating theater, they changed clothes and walked into the corridor of the operating room. A sense of emergency and haste hit them in the face.

A Circulating Nurse ran by with an armful of Red Blood Cell Suspension, moving in a hurry without saying a word of nonsense.

Arriving at the emergency Operating Room, Wu Zong asked, "What kind of patient?"

"Highway car accident, even with a seatbelt on, they were ejected," the Resident Chief of Hepatobiliary Surgery said.

"How were they ejected while wearing a seatbelt?" Wu Zong was stunned.

"Don't know, that's what they said when the 120 Emergency Vehicle brought them in," the Resident Chief of Hepatobiliary Surgery said. "You can see the seatbelt marks on the abdomen, very obvious. I don't know how it happened either."

You encounter all sorts of bizarre things in Clinical practice, and since no one was at the scene, everything was just hearsay. Wu Zong didn't dwell on it.

"What's the condition?"

Wu Zong leaned in as he spoke.

"Wu Zong, why did you bring so many people?" the Resident Chief of Hepatobiliary Surgery asked.

"A crowd shows respect. Can we afford to have too few people at an intraoperative consultation for you, Jia Lao Zong?" Wu Zong said something nonsensical and leaned in to take a look.

The Resident Chief of Hepatobiliary Surgery was in the middle of resecting the spleen; he casually lifted the intestines and had his assistant use a large retractor to pull up the abdominal wall.

Just as Wu Zong had said before, the Retroperitoneal hematoma was severe.

"Tsk tsk, what does the scan say?" Wu Zong asked.

"It's on the film viewer. We only did a Fubu CT; the patient's blood pressure was almost gone, no other exams."

"You gave 10u of red blood cells during the surgery; I guess this life was saved."

While they were discussing the patient's condition, Ji Xiang walked to the film viewer and examined the images carefully.

On the film, the hematoma in the Retroperitoneal area was obvious, a big white patch where you couldn't really distinguish anything.

But Ji Xiang had the experience of using his keen insight hundreds of times, and his vision had improved tremendously without him realizing it. He could vaguely see something wrong.

Holding his breath and focusing his mind, Ji Xiang entered the System Operating Room.

"Teacher, I've seen the patient. It's a car accident trauma. The current diagnosis is Spleen Rupture and Hemorrhagic Shock."

"There is a detail in the patient's imaging. Did you see it?" The System NPC led Ji Xiang to the film viewer.

"Look, here. Do you see any problem?"

Ji Xiang's film-reading level was still slightly lacking; it hadn't kept up with the surgical level of the Urology Department and Interventional Department.

"Teacher, I can see a layer of membrane here, but I don't know what it means."

The System NPC nodded. "That's already not bad. This is a Urinoma, which means the Ureter is ruptured."

Immediately, the System NPC explained the relevant theoretical knowledge to Ji Xiang and took him to perform the surgery.

……

……

"The patient is unlucky; they were thrown out even while wearing a seatbelt. You can imagine how fast the car was going. But the silver lining in the misfortune was running into a 120 Emergency Vehicle transfer. They happened to encounter the accident, and the 120 Doctor was also anxious. They loaded the patient up right then and there, screaming all the way back."

"When they arrived, the blood pressure was 60. If they had been delayed on the highway for half an hour, the person would probably be gone."

"Then you still had time to do a CT."

"With an Indwelling Needle in, we were pumping in all sorts of colloids and crystalloids. Anyway, the operating room had to be prepared, so I had 10 minutes," the Resident Chief of Hepatobiliary Surgery said. "I envy you guys in the Urology Department; you never have to race against time. Going to other departments is just to insert a catheter."

"What are you saying? We have plenty of emergencies in the Urology Department, okay?" Wu Zong said, refusing to be outdone, as if having many emergencies were a good thing.

"Wu Zong, be careful," Zhao Zhe reminded him vaguely.

Wu Zong immediately deflated.

"Take a look, see if it needs to be opened," the Resident Chief of Hepatobiliary Surgery said. "I'm almost done here. If you don't open it, I'm closing the abdomen and getting ready to go down."

"I'll go take a look." Wu Zong wasn't in a hurry. He had glanced at it when he entered and felt it was fine.

Turning to look at the films, Ji Xiang stood in the best position.

Wu Zong didn't push Ji Xiang aside but stood next to him. "Left diaphragmatic rupture, splenic injury, Grade I left Kidney injury accompanied by Retroperitoneal hematoma."

"There's nothing for us to do. Let's observe for now."

"Wu Zong."

Just as Wu Zong finished speaking, he heard Ji Xiang's voice.

That damn sense of familiarity... No, there was no familiarity, but why were his hands and feet cold? Wu Zong was startled.

"What is this here? I don't understand it." Ji Xiang pointed his finger at the film on the viewer, making a "thump" sound. Wu Zong was almost scared into a supraventricular tachycardia.

"Here?" Wu Zong was puzzled, but that damn feeling of familiarity that should have come yet didn't was very uncomfortable. He dared not be negligent and hurriedly looked in the direction of Ji Xiang's finger.

It seemed there was a problem, yet it also seemed there was no problem.

Wu Zong frowned and pondered for a long while.

"It's nothing, consider it an artifact left behind from the hematoma, no need to worry about it," Lu Gang said, standing behind with an imposing air.

Wu Zong immediately felt confident, "Lu Zhuren is right, it is an artifact."

"Why do I feel like it's a urinoma?"
