Chapter 47: Cavernous Body Puncture (Part 2)

~12 min read 2,209 words

Gu Lao shook his head, signaling that he had no good solution either.

It was true that he was a medical Expert, but the Patient was a burn victim, and even Gu Lao was powerless. Coming to take a look was the most he could do.

"Use every possible means to save them. No matter how many difficulties there are, even if they are as big as the heavens, at least keep them alive for 3 days!" The person from the city said with a cold face and a stern voice.

Chen Yuanzhang looked bitter and said nothing.

Medicine is a branch of science, and objective science is science; it won't be so obedient.

Look at the state the Patient is in—3 days?

Chen Yuanzhang felt they wouldn't even last 3 hours.

Once the person is gone, he'll just have to wait to be scolded; what a stroke of bad luck, Chen Yuanzhang thought, his heart filled with gloom.

He came from a Clinical background; one look at the Patient's condition told him the Patient had already reached the gates of hell, perhaps even more thoroughly dead than someone in cardiac arrest, even if they still had a pulse, a heart rate, blood pressure, and could still scream in pain.

Large-area burns lead to massive exudation; fluids cannot be replenished, and the person will be gone very soon.

Miracles are impossible for patients like this.

"Gu Lao."

Ji Xiang came to Gu Lao's side and bowed slightly.

"Doctor Xiao Ji, you've come to participate in the rescue too." Gu Lao shook his head. "This has nothing to do with our Urology Department, go back."

"I just took a look at the Patient from afar and had an idea; see if it might work," Ji Xiang said in a low voice.

An idea?

Gu Lao frowned slightly, and Chen Yuanzhang's eyes lit up, but upon seeing that the speaker was a young man, his face immediately turned dark.

If it weren't for the people from the city being there, he would have loved to kick this ignorant kid into a corner.

"This is an emergency rescue, it's not..." Gu Lao stopped halfway, gave Ji Xiang a look, and then walked out.

Ji Xiang followed by Gu Lao's side. "Gu Lao, Cavernous Body Puncture; fluid can enter the Venous Sinus from the corpus cavernosum and then enter the Systemic Circulation."

Gu Lao's footsteps faltered, and he stumbled slightly.

This line of thinking...

Gu Lao had once heard a similar report from another big shot in the circle at an academic conference, but he had never performed it himself.

He never expected to hear similar words from the mouth of a Standardized Training Student.

"Don't talk nonsense!" Professor Zhou scolded.

"Doctor Xiao Ji, tell me more." Gu Lao stopped, raised a hand to stop Zhou Tianbo's words, and looked at Ji Xiang with focus.

Professor Zhou was stunned.

What Ji Xiang was talking about was only theory; it only existed in theory. He himself was considered an Expert in the Urology Department and had never heard of such a thing.

What is the boss doing! Professor Zhou wondered.

"Be careful when puncturing, especially..." Ji Xiang stated the key points that the System NPC had taught him in the System Operating Room.

"This..." Gu Lao hesitated.

However, the entire process Ji Xiang described had a very high degree of consistency with the report he remembered another person giving at the conference; it should be worth a try.

"Have you done it before?" Gu Lao asked.

After asking, Gu Lao was struck dumb.

He had been immersed in the Urology Department for forty years and had never done it; why would a little Standardized Training Student be able to!

He must have lost his mind, he must have!!

But if Ji Xiang couldn't do it, he could try it himself.

Try it?

Gu Lao was somewhat hesitant. After all, he was advanced in years, and he had never performed a Cavernous Body Puncture; what if there was a problem?

But that wasn't the most critical point; he had been drinking tonight, and Gu Lao worried his hand might slip during the procedure.

"Gu Lao, I can try," Ji Xiang said softly.

"Hmm?"

"You've done a circumcision and now you're getting arrogant?" Professor Zhou's expression was quite unpleasant. "You don't even have a medical license, get out of here."

Ji Xiang didn't move; he automatically filtered out Professor Zhou's insults and looked straight at Gu Lao.

Gu Lao and Ji Xiang locked eyes.

To be a Doctor, one must be brave!

Gu Lao suddenly recalled what an old Doctor had said to him when he was young and encountered a patient bitten by a dog.

A few seconds later, Gu Lao nodded. "Be steady, just bring out half the level of skill you use for a circumcision. The puncture is very simple; with me watching, it'll be fine. Try it!"

Professor Zhou was astonished, his face turning a bruised purple.

"Chen Yuanzhang, I'll take someone to try and see if we can establish Intravenous Access," Gu Lao turned and said to the Dean of the Second Affiliated Hospital.

"Gu Lao, what are you planning to do?" Chen Yuanzhang had already lost all hope and was starting to ponder how to shirk responsibility, making the leaders believe he had done his best.

As for Gu Lao, a urologist, what could he possibly do!

But out of respect for the senior, Chen Yuanzhang still asked.

"Perform a Cavernous Body Puncture." Gu Lao only greeted Chen Yuanzhang, then took Ji Xiang into the rescue room.

The Patient was exactly the same as the Simulated Human, only the person beside him had changed from the System NPC to Gu Lao.

Ji Xiang focused and began to prepare the items for the puncture.

"Gu Lao, Gu Lao." Chen Yuanzhang chased inside. "Did I hear that right? Cavernous Body Puncture? There's even this surgical procedure?!"

"Behind the corpus cavernosum is the Venous Sinus; we can use it for fluid resuscitation."

"!!!"

Everyone in the rescue room was stunned.

In emergency resuscitation, apart from cases of Kidney or Ureteral rupture, one rarely sees the Urology Department.

But through Cavernous Body Infusion—could it be they are finding a tube to pour liquid in through the urethra?!

This is far too unprofessional!

"Gu Lao, how will you do this?" asked Xu Zhuren of the Anesthesiology Department, his forehead soaked with sweat under his sterile cap, his voice tense and anxious.

"Let's try it; treat a dead horse as if it were a living one," Gu Lao said, uttering a vague remark.

Xu Zhuren tactfully stepped aside, watching Gu Lao with deep confusion.

Soon, Ji Xiang finished preparing the items; he stood in the operator's position, while Gu Lao stood in the assistant's position.

Seeing this scene, Zhou Jiaoshou felt a tightness in his chest.

This young man was too arrogant; although it was only a minor procedure, not a surgery, he was standing brazenly in the operator's position right next to his own boss!

This undoubtedly made Zhou Jiaoshou feel dissatisfied.

"Gu Lao." Ji Xiang turned slightly, politely making a gesture for him to proceed.

"We are in the middle of resuscitation," Gu Lao said earnestly. "You do it; pay attention to the sensation of loss of resistance when piercing the White Membrane."

Ji Xiang felt moved; as expected of an old Doctor and an old Expert.

It was clear that Gu Lao had never performed a similar procedure, yet he could immediately state the key points of the operation.

"Gu Lao, then I will begin."

After Ji Xiang finished speaking, he began the Skin Preparation of the surgical area.

The Patient was diagnosed with a Severe Burn, 35% third-degree, 50% second-degree, and Burn Shock.

Gu Lao knew that in similar situations, survival was difficult; aside from infection, it was mainly because Intravenous Access could not be established.

Infusing through the Corpus Cavernosum made sense logically, but this approach was far too wild!

He watched Ji Xiang's movements with focused attention, prepared to interrupt immediately if there was anything he was dissatisfied with.

Yet, after just one look, Gu Lao's brows locked tightly, and his eyes narrowed into slits.

The few remaining patches of intact skin on the Patient were their hope for survival; future skin grafts and the like would all depend on this.

During Skin Preparation, even the slightest injury would count as a major error.

However, Ji Xiang's movements during the Skin Preparation were gentle, without excessive demands for tidiness; this seemed like a flaw. Yet from Gu Lao's perspective, what Ji Xiang did was just right.

If this child's movements were any more, it might damage the skin; any less, and the Skin Preparation would not be clean, potentially triggering an infection.

Just right—these four words are simple to say, but truly achieving them is extremely difficult.

Gu Lao considered himself capable of doing this ten years ago, but now, heh.

Ji Xiang simply trimmed the hair residue, disinfected the skin around the surgical area, held the surgical area with his index finger and thumb to fix the posterior Corpus Cavernosum, and selected the area on the right side of the Corpus Cavernosum, lateral to the shaft, where there were no subcutaneous veins.

Gu Lao wanted to help, but Ji Xiang's movements were fluid and accomplished in one go, as if he had performed similar training countless times, making Gu Lao's eyelids twitch.

This is fucking incredible!

The next step was the key; Gu Lao had no time to be amazed and focused his attention to continue watching Ji Xiang's operation.

Without even looking, Ji Xiang reached back and picked up a No. 6 Syringe needle.

"Who is that?"

"It seems to be Gu Lao from the Urology Department."

"No, I mean the young man performing the procedure."

Behind them, people began to discuss.

Although the resuscitation scene was chaotic, everyone was focused on this patient, who was of the utmost importance.

Even the director of the Anesthesiology Department had failed; who else dared to step up!

But when a young man stood in front of the surgical area and began the procedure, almost all eyes were drawn to him.

People were surprised and couldn't help but mutter in low voices.

"What is he doing? The small veins up there are thin as can be; you can only try with a Scalp Needle. A No. 6 Syringe needle won't go in, right?"

"I don't know, I just heard the director say they were looking for the Interventional Department to see if an intervention would work."

"Can the Interventional Department do it? That's not how you place a central venous port either."

"Didn't the leaders say it? They must survive."

"Listen to them! Pfft, if that's the case, just have him stand at the entrance of the funeral home, and everyone could be brought back to life."

"Keep your voice down, Chen Yuanzhang is definitely going to get scolded, don't get in his line of fire."

Ji Xiang was fully focused and didn't hear what the people around him were discussing at all; he held the No. 6 Syringe needle and placed it at a 45-degree angle on the side.

It really might work, Gu Lao calculated, finding no errors in Ji Xiang's operation, and remained silent.

If there was anything wrong, it was that this young man started as if he had done it at least a hundred times; he was extremely proficient, which looked truly strange.

"What is he doing? He isn't actually planning to stick a needle in and then start an infusion, is he?"

"Yeah, can that thing work? It's not a blood vessel."

"Why isn't a single director speaking up?"

Seeing Ji Xiang holding the No. 6 Syringe and about to pierce, the crowd was stunned.

The current medical system is categorized very finely; you could say that every department is separated by a mountain.

It's not a blood vessel, and you can just start an infusion with one needle?

The crowd did not believe it.

"Chen Yuanzhang, I am not in the medical field, but I know that infusions must be in a blood vessel," the city leader said with a stern expression. "Isn't this just nonsense! That is used for urination; it is a place for things to come out, not a place for things to go in. Are you fucking bullying me because I don't understand!"

Chen Yuanzhang was also suffering bitterly.

"I'll go take a look, leader."

Pushing through the crowd, Chen Yuanzhang arrived behind Ji Xiang, and a surge of anger flared up in his chest.

Originally, Chen Yuanzhang thought Gu Lao and this young Doctor were looking for small arteries or veins.

No matter how small, as long as the liquid went in, he would have an explanation.

But!

But!!

But!!!

The number 6 needle in Ji Xiang's hand perfectly avoided all blood vessels and was inserted directly.

Is this supposed to be emergency resuscitation?

Could it be that he is actually planning to connect a water pipe and force the liquid in retrograde?

A rookie who doesn't understand the slightest bit of medical common sense!
