Chapter 482: So-called Clinical Experience
"Teacher Li, you are too polite."
At the same time, Ji Xiang and Li Zhuren spoke to each other very politely.
"After the program ends, even if you don't stay, Doctor Xiao Ji, you must come over to my side to exchange ideas more often," Li Zhuren said, patting Ji Xiang's shoulder firmly.
After saying this, Li Zhuren tilted his head slightly, looked into Ji Xiang's eyes, and said seriously with added emphasis, "Doctor Xiao Ji, before you, I didn't really believe in things like talent, but after seeing your super-selection surgery, I truly believe it."
Ji Xiang scratched his head.
"Speaking of which, Qin insisted on taking you as his closed-door disciple; when I first received the news, I thought it was completely unbelievable. Now that I see it, Qin... hahaha, I won't speak ill behind his back, but at the very least, Qin's eye for talent is very sharp; in terms of professional ability, he is considered a leader in the industry."
Every word Li Zhuren said was about professional work, and Ji Xiang understood what Li Zhuren meant.
"Teacher Li, then I will head back first," Ji Xiang said.
"Good, good, good. Let's communicate again when there is time."
Li Zhuren walked Ji Xiang to the entrance of the ward, holding Ji Xiang's hand in a moment of uninhibited enthusiasm.
Ji Xiang returned to the Hepatobiliary Surgery Department and saw Zhai Yuhan writing a Medical Record with Zhao Mengmeng. He stepped forward and asked, "Teacher Zhai, I'm back. Let me write it."
Zhai Yuhan looked up and glanced at Ji Xiang twice.
"Did the surgery over there go smoothly?" Zhai Yuhan asked.
"Smoothly."
Zhai Yuhan let out a soft sigh of relief.
Hepatobiliary Surgery is one of the departments that has relatively more contact with Interventional Surgery, and Zhai Yuhan knew how difficult these types of surgeries were to perform.
And the fact that Li Zhuren of the Interventional Department actually held Ji Xiang in such high regard gave Zhai Yuhan a different perspective on Ji Xiang.
It was only natural that she could no longer treat the person in front of her like an ordinary intern.
Zhai Yuhan had never imagined that Ji Xiang had actually performed such a major surgery in the interventional operating room; she wouldn't have even dreamed of it.
"Doctor Xiao Ji, sit down," Zhai Yuhan said. "Wearing Lead Clothing for interventional surgery is especially tiring. You should rest for a bit; there's no rush. By the way, I see that your Clinical experience is very rich. Were you often at the hospital during your internship and clinical rotations?"
"When I was at the Second Affiliated Hospital, I met a teacher from the Emergency Department. His level was very high, so whenever I had free time, I would follow him to watch and learn a bit more. Does that count as having some Clinical contact? I suppose so," Ji Xiang said with a smile.
"That is indeed Clinical experience," Zhai Yuhan said, making an ambiguous remark.
"What is?" Zhao Mengmeng had been listening with her ears pricked up, but Zhai Yuhan stopped abruptly after saying half a sentence. She was a bit confused, and the question slipped out.
"Hehe." Zhai Yuhan compared the bewildered Zhao Mengmeng with the calm and composed Ji Xiang, knowing in her heart that their understanding of this matter was completely different.
"Let me give you an example," Zhai Yuhan said, putting down her work to take a break. "A few years ago, when I was an intern, I encountered a Patient."
"The Patient was diagnosed with a herniated disc and had radiating pain in the lower limbs. The preoperative symptoms were very severe; the left lower limb was in so much pain that they couldn't move. At the time, my teacher admitted them to the hospital, and later, surgery was performed."
"The surgery went very smoothly, and the nerve root decompression was ideal. In the first week after surgery, the symptoms improved significantly. From my perspective, the Patient would be able to be discharged in a few more days. Our Affiliated Hospital is not like this place; it doesn't matter if they stay a few extra days to recover."
"However, on the eighth day after surgery, the Patient complained of significant pain in the left lower limb, and the Clinical physical examination took a sharp turn for the worse. So now, the question is, how would you judge this?"
As Zhai Yuhan spoke, her gaze turned toward Ji Xiang.
Ji Xiang didn't answer, just smiled slightly. Zhao Mengmeng said, "Teacher Zhai, how was the physical examination of the Patient?"
"During the physical exam, the Patient's muscle tone was Grade 1, even worse than before the surgery. All kinds of tests were positive; they would scream in pain at the slightest touch."
"Could it be a reaction that appeared after the edema subsided due to nerve root damage? Was an MRI done?" Zhao Mengmeng continued to ask.
"The MRI showed no hematoma or nerve compression, the CT showed the pedicle screw fusion device was in a standard position, and the electromyography showed no evidence of nerve damage," Zhai Yuhan listed all the major examinations in one breath.
Now Zhao Mengmeng was stumped; she had no idea what the Patient's situation was.
The Auxiliary examinations were fine, yet the Patient's self-reported symptoms were very severe. In common terms, the physical examination and the objective examinations were contradictory.
Seeing Zhao Mengmeng's bewildered expression, Zhai Yuhan asked Ji Xiang, "Doctor Xiao Ji, what do you think?"
"Teacher Zhai, the Patient is probably faking the illness," Ji Xiang said calmly.
"Oh? Why is that?" Zhai Yuhan asked.
"Within a week after surgery, the Patient was worried about recovery and was very cooperative. Once a week passed and they knew they were fine, they started faking the illness to try and extort some money. Similar situations are not uncommon," Ji Xiang laughed.
"That can't be right, how could a Patient fake an illness?" Zhao Mengmeng frowned, unconvinced.
"I heard about a case from Bai Chuzhang of the Medical Affairs Department at our hospital. The Patient had tetracycline teeth and was receiving Root Canal treatment at our hospital's Oral Department. After the treatment, they claimed the tetracycline teeth were caused by the Root Canal treatment."
"..." Zhao Mengmeng looked at Ji Xiang innocently, completely failing to understand what Ji Xiang was talking about.
Zhai Yuhan knew very well that this top student had always lived in an ivory tower; they had no idea how sinister human nature could be, nor how difficult Clinical work could be.
She looked at Ji Xiang with great interest, not expecting that Ji Xiang was actually quite familiar with the Director of Medical Affairs at his hospital.
"How did you handle this matter, or rather, resolve it?" Zhai Yuhan asked.
"That Patient was dressed in rags and looked very poor. They came in crying, saying their family situation was bad. Bai Chuzhang said that tetracycline teeth mainly appeared in the late seventies and early eighties; at that time, tetracycline was very rare, and very few people could afford to use it."
"Therefore, this illness is a 'disease of the wealthy'."
"Bai Chuzhang didn't believe the Patient could be that poor, so he had someone keep an eye on them. Sure enough, one time the Patient came riding a Shared Bicycle. After chatting for a whole morning, they left, rode the bike out of the hospital, and then drove away in an Audi A6."
"..." This time, even Zhai Yuhan was a bit stunned.
It stands to reason that while someone driving an Audi A6 isn't necessarily super wealthy, it's normal for them to have a little money.
Would such a person use an absurd reason to keep causing trouble for the hospital just to extort a little money?
"That can't be," Zhao Mengmeng said in surprise.
"That's what I heard. Later, it seems Bai Chuzhang happened to be doing a Clinical inspection, and when a doctor made a mistake and went to the Medical Affairs Department for a disciplinary talk, he assigned that Patient to that doctor to be responsible for."
"Responsible for?"
"It means accompanying the person who is complaining to chat; they have no choice but to chat."
"What a rogue method..." Zhai Yuhan hadn't expected that Bai Chuzhang would come up with such a tactic.
The person complaining came on a bicycle and definitely didn't want people to know they were actually wealthy. With a doctor constantly pestering them, they had no way to go and drive the Audi A6 away.
When a rogue meets a rogue, it's a test of who has more patience.
How could someone complaining about a hospital have more patience than a doctor who is already undergoing a disciplinary talk? Zhai Yuhan already knew the result.
