Chapter 484: Content with Small Wealth

~8 min read 1,445 words

Only after Ji Xiang recounted a few stories about himself and that Mo Laoshi did Zhai Yuhan suddenly snap to her senses.

How could she have actually been listening to an Intern give a lecture?

This Intern before her was different from the others; this was something Zhai Yuhan could understand, and also something she acknowledged.

But Ji Xiang was so young; judging by Zhai Yuhan's social and Clinical experience, Ji Xiang should have been extremely lacking in this area.

Yet, when chatting about such topics, Ji Xiang did not fall behind in the slightest, which left Zhai Yuhan feeling a bit unconvinced.

But it was only a feeling of being unconvinced.

In just a few days, Zhai Yuhan had already changed her attitude toward the Intern participating in the reality show, though she had not yet realized it.

"Doctor Xiao Ji, what left the deepest impression on you when you first arrived at the hospital and encountered Clinical work?" Zhai Yuhan asked, changing the subject.

She wanted to steer the conversation back to a rhythm she was familiar with.

"The deepest impression? Of course, it's the Internal Medicine doctors writing Medical Records and taking patient histories; the Surgery Department doctors performing surgeries and..."

"And what?"

"Driving." Ji Xiang scratched his head.

"I find Yang Zhuren to be very serious when performing surgery." Zhao Mengmeng had never been to the Clinical side and knew nothing about this, mistakenly taking Yang Zhuren's performance while the production crew was present as the norm.

"Then tell me about the time that left the deepest impression on you," Zhai Yuhan said.

"On my first day of Standardized Residency Training in the Clinical department, I heard a teacher tell a Patient that scars are a man's most dazzling badge, the shiniest one of all... Hahaha."

Zhai Yuhan knew this meme; she frowned slightly and gave Ji Xiang a look that said, "That's it?"

Ji Xiang thought for a moment. "I remember one time I went to the Nan Ke Yi Yuan with two teachers to perform surgeries. Usually, I acted as the assistant, and the two of them took turns as the Surgeon. Since it was a holiday, there were many children getting Phimosis Surgery, over a dozen surgeries a day; we did as many as 22 in one day."

"One day, Wang Laoshi seemed a bit tired. He didn't want to perform surgery, so he sat on the side and refused to scrub in, telling Mo Laoshi to take his place. After Mo Laoshi asked twice, Wang Laoshi said this was a test for him."

"A test? What kind of test?" Zhai Yuhan asked.

"Whoever performs the surgery gets the surgical fee for each case," Ji Xiang explained. "Wang Laoshi said this was to let you earn a bit more money. The two of them are in the same group and are used to joking around; it wasn't that he really wanted to do less work and earn more money. Mo Laoshi said he had enough money and didn't need it."

"Earning money." Zhai Yuhan hesitated; she really couldn't understand what the problem was here. Even if he were the God of Mount Akina, it would probably be hard to drive in a way that could be spoken of openly.

"Wang Laoshi said..." Ji Xiang imitated Wang Daxiao's mannerisms, raising his hand to stroke his hair. "Hongkongdoll, a little rich girl with 15 million in assets filming on P-site, can you believe it? Look at her, then look at yourself. Content with small wealth, what a loser."

"..."

"..."

Zhai Yuhan and Zhao Mengmeng were both stunned. Zhai Yuhan truly didn't know P-site, while Zhao Mengmeng knew it but didn't know who Hongkongdoll was.

Ji Xiang also hesitated; although Zhai Yuhan had brought up the topic of "driving," it was still somewhat inappropriate.

The joke had gone a bit too far.

How could he just casually repeat the words of an old scoundrel like Wang Laoshi?

"Emmmm..." Ji Xiang wanted to change the subject.

Zhai Yuhan took out her phone and started searching according to what Ji Xiang had said, and Zhao Mengmeng leaned over to look along with her.

Ten-plus seconds later, their expressions turned slightly strange.

There were plenty of Surgeons who spoke without restraint on the operating table, but most of it was that kind of straightforward "driving" that didn't require any brainpower.

Although she understood what Ji Xiang was talking about, she didn't quite grasp it and still had to look it up.

She steadied her mind; she really wanted to win against Ji Xiang once.

Suddenly, Zhai Yuhan had a flash of inspiration and said with a smile, "Mengmeng hasn't been to the Clinical side and isn't used to it. Doctor Xiao Ji, let me test you with a Clinical case to see if you can answer it."

"Sure." As soon as Ji Xiang heard there was a Clinical case, he immediately sat up straight, placed his hands on his knees, and his entire demeanor changed abruptly.

Zhai Yuhan was a bit surprised by Ji Xiang's attitude upon hearing about a case, but she still brought up the case from her own career as a Doctor that had left the deepest impression and was the easiest to misdiagnose.

"Patient is male, 37 years old, admitted with the chief complaint of 'weakness in both lower limbs for 2 days, aggravated by paralysis of both lower limbs and amaurosis for 5 hours'."

"Amaurosis?" Zhao Mengmeng caught the key point and whispered.

She also wanted to perform well; after all, she had been a top student since she was a child, and she hadn't expected to be crushed into dust by Ji Xiang inadvertently while participating in a reality show.

Amaurosis is a symptom with significant diagnostic importance.

It often indicates that an ipsilateral carotid artery is about to become occluded, so it is often regarded clinically as a precursor to carotid artery disease; 70% to 97% of patients with transient amaurosis can be predicted to have carotid artery disease.

But since Teacher Zhai Yuhan was able to bring out this case to "test" Ji Xiang, it shouldn't be that simple.

Upon hearing the chief complaint, Ji Xiang's expression did not change in the slightest; he quietly watched Zhai Yuhan, waiting for the history of the present illness.

"2 days ago, the patient developed weakness in both lower limbs. The systolic blood pressure was measured at 240mmHg. After taking hydrochlorothiazide and losartan orally, the systolic blood pressure dropped to 160mmHg."

"The weakness in both lower limbs was slightly relieved, and the patient did not go to the hospital for treatment."

"5 hours ago, paralysis of both lower limbs occurred, accompanied by amaurosis. One week ago, there was paroxysmal headache, without vomiting or fever. Past history of hypertension for 3 years, intermittent medication, no antihypertensive drugs taken in the last 10 months. Denies history of 'diabetes' and 'peripheral vascular disease'."

"Admission physical examination: blood pressure 152/86mmHg, clear consciousness, no murmurs heard on cardiac auscultation, systolic murmurs audible in the right subclavian and carotid artery areas, bilateral radial artery pulses normal, muscle strength of both lower limbs grade 0, muscle strength of both upper limbs grade 4, bilateral dorsalis pedis artery pulses normal, bilateral pathological signs negative."

"Ji Xiang, Mengmeng, tell me your thoughts."

After hearing the history of the present illness and past history described by Zhai Yuhan, Zhao Mengmeng already had the answer. This question was not difficult, at least not for a Doctor who knew the definition of amaurosis.

"Teacher Zhai, it is cerebral ischemia caused by carotid artery stenosis," Zhao Mengmeng said with full confidence.

Zhai Yuhan was noncommittal and looked at Ji Xiang.

Ji Xiang pondered, thinking silently for nearly ten seconds.

"Teacher Zhai, what did the Head CT report?" Ji Xiang asked.

"Head MRI indicates: acute Cerebral Infarction in the bilateral parietal-occipital lobes; small ischemic lesion in the left semiovale center."

Zhao Mengmeng was startled.

According to her judgment, the patient's head MRI should have shown ischemic lesions; how could it be an acute Cerebral Infarction?

But murmurs could be heard in the carotid artery area, and there was amaurosis; these were all signs of carotid artery disease.

"Was an Ultrasound of the carotid artery not performed?" Ji Xiang asked.

"It was performed, and it was normal," Zhai Yuhan said with a slight smile, stating a key point.

Ji Xiang was also stumped; his thoughts were the same as Zhao Mengmeng's, but Zhai Yuhan had clearly dug many pits in the case study questions.

Holding his breath and focusing his mind, Ji Xiang arrived at the System Operating Room.
