Chapter 517: 509 Boss Peeed

~6 min read 1,096 words

Hearing Director Xia say that, the patient's family also showed a puzzled expression.

It didn't sound like nonsense or placating himself…… But then he glanced at Zheng Ren again—indeed, he was a young doctor.

That's strange.

Director Xia's reputation is known to many ordinary citizens in Hai Cheng; she is one of the top gastroenterology experts in the whole city.

How could she praise this young doctor so highly?

"Chief Zheng, the patient with acute severe pancreatitis—his amylase has basically returned to normal." Director Xia glanced at the films while bringing up that patient from two days ago to Zheng Ren.

"Mm, it's your expertise that cured him." Zheng Ren answered offhandedly, his mind full of related diagnostic and differential diagnostic information.

"Thanks to you." Director Xia didn't mind Zheng Ren's perfunctory reply, chuckling: "How did you diagnose this patient, Chief Zheng?"

"It looks like a large intra-abdominal abscess, but I don't think so; I'm leaning toward a colonic diverticulum."

"A diverticulum?" Director Xia paused.

"Hmm." Zheng Ren picked up the patient's lab tests from the Emergency Department and said, "Director Xia, look—the patient's blood count shows no major abnormality. If it were a large intra-abdominal abscess, there would have to be corresponding changes in the blood tests."

Director Xia fell into thought.

"Of course, this is just a suspicion. The patient is elderly, so his body might not react strongly. But if we diagnose a colonic diverticulum, everything can be explained naturally." Zheng Ren pointed at the images, launching into professional radiology terminology to lay out his thoughts, speaking for a full three minutes.

Su Yun's brows were tightly knit, his head slightly lowered; through the black hair across his forehead, he watched Zheng Ren's eloquent side profile.

On that normally plain face, confidence shone, slightly ostentatious yet not excessive.

Just who gave him that confidence?

Su Yun felt somewhat lost.

Such differential diagnosis requires skill, and even more, experience.

In terms of skill, Su Yun does not think…… he himself does not believe Zheng Ren is much stronger than him. So why is he so confident?

The same question also arose in Director Xia's mind.

Strange!

"Chief Zheng, I think what you just said might be because the patient is older—his blood might not react quickly enough." Director Xia and Su Yun shared a similar view, believing the likelihood of a large intra-abdominal abscess was higher.

Zheng Ren smiled and said, "I haven't seen a similar case at our City First Hospital, but the literature mentions it. An article by Dr. MoliereS published in Surgery described a similar case, very like this patient."

Su Yun suddenly understood—Zheng Ren had read similar reports, which allowed him to be fairly certain.

It seems the road to catch up through effort has gotten much longer.

But it's strange—normally I never see Zheng Ren reading journals; maybe he read them back in school?

Director Xia still held to her view, asking: "Similar cases can't prove……"

"Director Xia, look here." Zheng Ren's face shone with confidence; he recalled the case report he'd just seen, then worked backward, and everything suddenly became clear.

Zheng Ren took out a pen, pointed its tip at a spot on the abdominal CT, and said, "Here, the fluid-level image shows a slight change. The CT scan data is processed to obtain each voxel's X-ray attenuation or absorption coefficient, then arranged into a numerical matrix. Based on the attenuation pattern of that matrix, I judge the density……"

The endless stream of professional jargon left Director Xia bewildered.

Although she is the head of gastroenterology, well-known in Hai Cheng with rich clinical experience, that doesn't mean Director Xia is familiar with CT to Zheng Ren's level.

"Boss, he peed! He peed! The patient has successfully urinated!" Professor Rudolf Wagner's excited voice roared like a lion's, coming from the other end of the hallway.

"Su Yun, make Fugui'er shut up!" Zheng Ren frowned, speaking displeased.

Soon, the professor, unable to contain his excitement, hurried to the office.

"Boss, boss, he peed!" the professor whispered excitedly.

"Fugui'er, it's not the boss who peed—it's the patient's normal postoperative urination." Zheng Ren, somewhat displeased, corrected: "Isn't peeing normal? Making a big fuss over nothing."

Zheng Ren then said flatly, "Director Xia, let's continue."

Professor Rudolf Wagner glanced at the images and offhandedly said, "Boss, you because of an intra-abdominal abscess…… no, the density doesn't look like an abscess."

"Fugui'er, what do you see?" Zheng Ren, unable to tolerate the professor's chatter any longer, skipped explaining to Director Xia and asked the professor instead.

"Boss, I think it could be a colonic diverticulum or maybe an abscess—this differential……"

"I think it's a colonic diverticulum. If you take the image, reverse‑engineer the numerical matrix, add a reconstruction, and look from the perspectives of transmission and absorption, it becomes much clearer." Zheng Ren said.

"Boss, bravo!" Professor Rudolf Wagner made an exaggerated expression, pointing at the CT film with his finger: "I understand what you mean—it's definitely a diverticulum, without a doubt a diverticulum!"

Director Xia simply couldn't follow the exchange between Zheng Ren and Professor Rudolf Wagner, looking utterly baffled.

Su Yun's brows stayed locked; after a dozen seconds, he showed an expression of sudden realization.

The patient's family was even more at a loss, wondering why there would be a foreigner speaking such fluent Chinese at City First Hospital.

"Boss, I think God not only kissed your hands but also gave you a one‑of‑a‑kind brain." Professor Rudolf Wagner continued his praise, utterly unashamed: "The patient has urinated—I saved a urine sample for you to look at? It looks like Munich's finest beer."

Uh……

Zheng Ren couldn't comprehend the professor's excitement, gave him a disgusted look, and kept him at arm's length.

"I've got things to do here; you go get busy." Zheng Ren said dismissively.

The professor was about to defend himself, still wanting to argue, but seeing Zheng Ren's imposing, anger‑free expression, he felt intimidated and slunk away.

"Director Xia, the diagnosis is fairly clear; I think surgery is needed. I don't recommend laparoscopy because the diverticulum is too large—open surgery would be more reliable." Zheng Ren said.

Director Xia didn't understand Zheng Ren's talk of numerical matrices or reverse engineering; she just felt it made sense. She stared blankly at the abdominal CT plugged into the viewer, thinking, perhaps only a Philips engineer who started studying medicine could pull this off.

"Director Xia?" Zheng Ren asked again.

"Huh?" Director Xia snapped back, smiled, and said, "The younger generation is formidable—since the professor and you agree, let's go with surgery."
