Chapter 544: with translation of chapter title

~7 min read 1,279 words

Director Kong was a little surprised.

Although I say so myself, I am the head of the Interventional Radiology Department at a large tertiary hospital in the capital city; my expertise in interventional imaging, while not among the very best nationwide, is still firmly in the top ten.

If even I can't understand it... Could this kid Zheng Ren have overturned a certain area of interventional medicine?

Although I don't really believe Su Yun's claim, Director Kong thought about it and realized this is a procedure that Professor Rudolf Wagner is preparing to submit for a Nobel Prize; if it were too simple, it wouldn't be credible.

He didn't think about anything else; Zheng Ren had already taken out the film and placed it on the viewer.

Liver, nuclear magnetic resonance, diffusion?

Director Kong took a glance and was stunned.

The liver only undergoes a plain CT scan, 64-slice enhanced CT, plus plain and enhanced MRI. Ultrasound is rarely used because its resolution is insufficient.

No one has ever used magnetic resonance diffusion to examine the liver.

This is getting interesting; Director Kong crossed his arms and stood at the back, his eyes narrowing.

"Old Gao... Lao Gao, where should we puncture on this film?" Zheng Ren asked.

Gao Shaojie had already looked at the film, had a plan in mind, took out a pen, and marked a spot on the diffusion MRI.

"Teacher Zheng, it's here." Gao Shaojie said.

Zheng Ren nodded; Gao Shaojie indeed lives up to being a top graduate from a prestigious school, mastering the basics of using diffusion MRI to determine the TIPS surgical approach in such a short time.

But it's only basic; the point he mentioned still has some minor issues.

"Old Gao, this spot is usable, but not the best." Zheng Ren said: "By using and not using diffusion-sensitive gradient pulses and subtracting the two, what remains is the signal reduction caused by protons undergoing diffusion in the direction of the gradient pulse—that is, the image formed due to differing diffusion coefficients between tissues; artifacts can appear in this process."

"Artifacts?" Gao Shaojie pondered.

Director Kong looked bewildered.

For a clinical doctor, being able to read the images is already impressive; many doctors only look at the test reports on the films.

And what Zheng Ren is saying about gradient pulses... proton movement... Is this really within the realm of medicine?

"You also performed surgery; during the previous TIPS procedure, the third patient experienced this situation. During surgery, we were looking for the puncture point under X-ray, so this position isn't impossible, just the success rate is slightly lower."

"Well..." Gao Shaojie felt a bit discouraged.

Regarding this film, he believed the spot he had chosen was definitely fine.

Because he had arrived the previous evening, he first examined the patients, then studied four diffusion MRI images throughout the night.

But he never expected that Zheng Ren would reject it again.

However, Gao Shaojie quickly regained his spirits, recalling the patient Zheng Ren had mentioned.

Gao Shaojie had seen all the patients on whom Zheng Ren performed TIPS surgery; he knew them well, and without much effort, the image data of that patient appeared in his mind.

"The signal strength of the gradient pulse is large, which can cause errors in image interpretation. Here, we need to reduce the gradient; I remember mentioning this to you." Zheng Ren tapped the film with his pen as he spoke.

Gao Shaojie felt a slight pain in his radial styloid, as if Zheng Ren were tapping his wrist with a hemostatic clamp.

Contemplating,

pondering hard,

meditating.

Zheng Ren wasn't in a hurry either; after a few minutes, he said again: "The correct puncture range for this patient is here."

As he spoke, he lightly drew a 0.5 cm diameter range on the film with his pen.

"Diffusion is a vector, having both magnitude and direction. Regarding the signal strength issue, you need to practice more and observe more; only then can you tell at a glance how to adjust and correct." Zheng Ren instructed earnestly, and Gao Shaojie nodded repeatedly.

Director Kong sighed.

"Director Kong, are you alright?" Su Yun asked with concern, not in a provocative tone.

"I'm fine." Director Kong waved his hand, "Su Yun, do you know how?"

Su Yun laughed, blew a breath, her black hair fluttering in front of her forehead, "Of course, after hearing it twice I'll get it."

"Then I'll give you a task." Director Kong wasn't polite either, "When you have time, teach our department's staff."

"Director Kong, you really aren't being polite." Su Yun said, "I'll tell you, the New England Journal has already recorded this paper; it will be published in the next two days." She added, "Fugui'er is waiting to take this to contend for a Nobel Prize."

The implication is that this is a Nobel Prize–level surgery.

Director Kong, worldly-wise, knew Su Yun was joking, and smiled: "Anyway, if you go to the capital city, there's nothing much to do."

"Huh? Go to the capital?" Su Yun blinked in surprise.

"I just consulted Boss Zheng; the 912 Hospital has sent personnel to support the Emergency Department of Haicheng First Hospital, and he's going to the capital." Director Kong said: "You wouldn't not want to go, would you? After all these years, returning home in glory isn't enough to satisfy your vanity?"

"Well, look at what you're saying." Su Yun's expression changed instantly.

"How many people can go?" Su Yun asked.

Director Kong was puzzled, "How many people?" The hospital only needs Zheng Ren, and of course Su Yun can go too—no problem. Back when he graduated as a master's student, many hospitals in the capital were vying for him; he was quite a hot commodity.

"How many more people?" What does that mean?

Su Yun caught a glimpse of Director Kong's expression from the corner of her eye, understood, and said: "Director Kong, my boss actually has his own instrument nurse."

As she spoke, she narrowed her eyes.

Everything goes without saying.

Director Kong stared, then after a few seconds he said slowly: "Su Yun, Boss Zheng really does have his own dedicated instrument nurse!"

"Yeah, wasn't that already said?"

"I thought you were bragging." Director Kong felt surprised inside and uttered a swear word.

"Which Nobel laureate doesn't have an assistant or something? Let me tell you, Director Kong: if the boss goes, I can't go up; the boss won't say anything. But if the instrument nurse can't go up, he'll have to come back the next day—do you believe me?"

"..." Director Kong hadn't expected this situation, and he was taken aback.

But it's not a big deal to lose one or two spots; this involves that big piece of meat called the Nobel Prize. If I don't seize it first, later many will covet it.

By then, let alone the instrument nurse; even acquiring Haicheng First Hospital and making Zheng Ren its director would be trivial.

"No problem." Director Kong said

While speaking, he lifted his head and glanced at Zheng Ren. Zheng Ren was telling Gao Shaojie about the second piece, and he himself couldn't even understand what he was saying…

Ten thousand alpacas galloped through Director Kong's heart.

He had originally planned to finish speaking today, confirm the schedule, and rush back today.

But seeing Zheng Ren explain the piece, Director Kong's heart itched unbearably; he had to see it himself to feel reassured.

He pondered for a moment, then picked up his phone.

"Hello, it's me."

"Mm, I'm not going back today; help me change the Airplane Ticket to the early flight the day after tomorrow."
