Chapter 121: Familiarizing with Obstetrics Instruments

~8 min read 1,417 words

"Good morning, Zhai Director." Everyone greeted her one after another.

The one who came in was the Obstetrics Department Director, Zhai Lin, around fifty years old.

She kept her figure well, her steps steady and forceful, her short hair combed immaculately, her expression stern yet not without warmth.

Her gaze swept around the room, carrying an unquestionable air of authority.

When she spotted the new faces of Wang Chuan and Pei Nanli, she gave a slight nod of acknowledgment.

"Good morning, everyone." Zhai Lin's voice was clear and steady: "Let's begin the shift handover."

A senior attending physician who looked somewhat weary but still in decent spirits immediately stood up, flipped open the record book, and began reporting at a measured pace:

"Director Zhai, colleagues, last night we admitted three expectant mothers, performed four deliveries, and one emergency cesarean section. There are currently thirty-two hospitalized patients.

New elective cesarean sections: three.

At 10:30, Surgery Room 2: Bed 18, Sun Mi, 39+1 weeks gestation, G1P0, first-time mother, complete placenta previa, no active bleeding. Blood preparation has been notified to the blood bank......

Planned induction of labor: two patients......

Awaiting spontaneous labor observation: four patients......

Key patients are as follows: Bed 3, Zhang Xiaomeng, 39+2 weeks gestation, G1P0, first-time mother, gestational diabetes mellitus, under insulin control. Admitted at 22:00 last night due to irregular contractions.....

Postoperative patient: Bed 25, Zhao Yangyang, 38+1 weeks gestation, G1P0, underwent emergency cesarean section due to fetal distress, returned to the ward at 02:30 this morning. Newborn Apgar score 9-10, transferred to Pediatrics for observation, mild asphyxia after resuscitation......

Newborn nursery: a total of seven newborns admitted, including one premature infant at 34 weeks, weighing 2100g, in an incubator, breathing steadily.......

Critical lab values: none. Blood transfusion: none. Resuscitation: none. Deaths: none. Special events: none. Handover complete."

Director Zhai Lin listened very carefully, jotting down key points swiftly in her ward-round notebook.

Once the night-shift doctor finished reporting, she looked at the Head Nurse: "Is there anything to add or any special points to note on the nursing side?"

The Head Nurse added details on several mothers' diet, emotional state, breastfeeding attempts, and key points of jaundice monitoring for individual newborns.

Zhai Lin noted each one down.

"Good, the situation is basically clear." Zhai Lin closed her notebook, her gaze sweeping over the group once more, lingering on Wang Chuan for an extra second.

"Now we'll split into groups for ward rounds. I'll lead one group to focus on the patients handed over last night, the newly admitted, and the postoperative patients. Attending Physician Qian will lead the other group for the relatively stable mothers. Interns Wang Chuan and Pei Nanli, you're with my group."

"Yes, Director Zhai." Wang Chuan and Pei Nanli answered in unison.

The group first arrived at the bedside of Ms. Zhang Xiaomeng in Bed 3.

The night-shift doctor and primary nurse briefly introduced the overnight situation.

Director Zhai Lin stepped forward personally, her voice gentle yet professional: "Ms. Zhang, how are you feeling? Does your belly still hurt? Is there anywhere that feels uncomfortable?"

While asking, she gestured for Wang Chuan to come closer and observe.

"Good morning, Director. It doesn't seem to hurt much this morning…" the mother appeared somewhat anxious.

"Don't worry, this is very common." Zhai Lin soothed her, while nimbly putting on gloves, "Come, let me check the cervical dilation for you."

She signaled to Wang Chuan and Pei Nanli to watch her standard procedures and communication techniques with the mother.

After the examination, Zhai Lin analyzed for Wang Chuan and the doctors in the group: "Cervical dilation progress is stagnant, uterine inertia. For GDM gestational diabetes mothers, cervical condition assessment is very important....."

She also instructed the primary doctor on points to note.

Wang Chuan listened intently. In his past life he was an Emergency Department doctor, so he still understood the basic obstetrics knowledge.

He had been more like a sponge just now, absorbing Zhai Lin's approach and rhythm in handling obstetrics cases.

The entire ward round lasted nearly an hour.

She brought Wang Chuan and Pei Nanli to the Obstetrics Department Demonstration Classroom, pointing at a row of instrument cabinets and several models along the wall:

"Wang Chuan, obstetrics is different from emergency medicine; the basics must be solid. These are the most commonly used instruments and models in obstetrics. Familiarize yourself with them first."

"You need to know what each item is called, what it's used for, how to hold it, and the basic operating procedure. Once you're familiar with them, if you haven't finished your shift yet, come find me at the third-floor Outpatient Clinic."

"Wang Chuan, come here a moment!" Zhai Lin called Wang Chuan outside the Demonstration Classroom again, "Thank you for helping Dr. Shen."

"Director Zhai, it wasn't just me helping. There were also interns like Wang Jia, Li Wei, and Luo De."

"Alright, I've remembered them all. Go study."

Wang Chuan turned and entered the Demonstration Classroom, where Pei Nanli was standing in front of an advanced delivery model, holding a Simpson obstetric forceps in her hand, her brow slightly furrowed, seemingly studying how to lock them together correctly.

The air instantly fell silent; sunlight streamed through the window, illuminating the floating dust motes in the air.

Pei Nanli opened the instruction manual, compared it with the diagrams, and tried again to lock the left and right blades together. Her movements were a bit clumsy; the two pieces of metal made a faint scraping sound and failed to mesh perfectly.

Wang Chuan, on the other hand, walked naturally toward the instrument cabinet as if returning to his own home: 'Pei Nanli, let me show you how I blow your mind.'

He didn't go straight for the obstetric forceps Pei Nanli was studying, but headed first for the fetal heart monitor.

His movements flowed like water, as familiar as operating his own mobile phone......

Although Pei Nanli had her back to Wang Chuan and was focused on the forceps in her hands, her peripheral vision was involuntarily drawn to Wang Chuan's fluid, precise operations.

Her motion of locking the forceps paused again. What on earth did this guy do in the Emergency Department? Even the obstetrics instruments he's this skilled with?

Wang Chuan set down the fetal heart monitor and picked up the Martin's pelvimeter.

He didn't look at any instructions; his fingers deftly adjusted the caliper width, then walked over to a pelvic skeleton model.

He simulated measuring the interspinous diameter (IS) and intercristal diameter (IC), and so on, his movements as standard as a textbook, even subconsciously mimicking the motion of recording data.

Pei Nanli finally couldn't help herself, slightly turning her head to watch Wang Chuan's flawless set of operations, her fingers gripping the forceps tightening slightly.

She remembered that when she first encountered this measuring instrument, she had spent half a day just figuring out the front and back of the calipers and the placement points, and had even had her angle corrected by the teacher.

What does Wang Chuan have that I don't...... His grades are better than mine, but only by a little.

From freshman to senior year. Basically every day during the day shift he studied with me in the library. What on earth did he study?

I didn't pay attention to what books he was reading back then. Could he have been studying twice as hard at night? ~~

Wang Chuan seemed completely immersed in his own rhythm; he set down the measuring instrument and walked toward the speculum display area.

He picked up the large, medium, and small sizes one by one, checking whether each opened and closed smoothly and whether the edges were smooth without burrs. Then he picked up a medium-sized speculum and walked over to a gynecological examination model.

His simulated grip, lubrication, insertion, and exposure steps were quiet, efficient, and professional throughout.

Pei Nanli completely stopped her movements, turned around, crossed her arms, and fixed her cold eyes motionlessly on Wang Chuan.

During her examination practice, her instructor had repeatedly emphasized gentleness and stability; while she considered herself proficient, seeing Wang Chuan's technique now made her finally understand what it meant to handle heavy tasks with effortless ease.

This guy... does he turn into a robot once he hits fifth year? Program fully loaded?

As if unaware of Pei Nanli's scrutinizing gaze, Wang Chuan set down the speculum and finally walked toward the area where Pei Nanli had just been, heading for the delivery instrument pack.
