Chapter 536: You Are the Real Teacher
Miao Zong took a look; it was the patient who had been hospitalized yesterday for Appendicitis and had chosen conservative treatment.
He said apologetically, "Doctor Xiao Ji, wait a moment."
"Teacher Miao, please go ahead with your work." Ji Xiang smiled, standing quietly against the wall as if he didn't exist.
Miao Zong asked, "What's wrong?"
"My mother is in severe pain, and I touched her; her forehead is burning hot."
"Let's go take a look." Miao Zong strode toward the patient's room.
The patient's condition was poor; Miao Zong reached out to touch her, and the body temperature felt to be over 38 degrees Celsius. Having the patient lie flat with her legs curled, Miao Zong began the physical examination.
There was no tenderness in the upper abdomen, but the patient had very obvious pain in the lower right abdomen.
It was just that the patient's fever was very pronounced; as soon as the quilt was lifted, one could feel a wave of heat rising, and touching her skin felt scalding.
At least 39 degrees. Miao Zong frowned and looked at the Family Member, "You come here."
After saying this, he turned around and was startled to see Ji Xiang following right behind him.
"Doctor Xiao Ji, you can just go wait in my Duty Room."
"Teacher Miao, I'll follow along and take a look." Ji Xiang looked at the patient, lost in thought.
Miao Zong had things to do and didn't stand on too much ceremony with Ji Xiang; he stepped out of the patient's room but didn't go far, standing at the doorway.
"Your mother is running a fever. Go to the Nurse and get a thermometer to check her temperature; don't use a temperature gun." Miao Zong gave special instructions, "I recommend emergency surgery."
"Doctor, my mother's health is poor..."
The Family Member said with a worried expression.
Miao Zong was also helpless; the pre-operative briefing and Anesthesia notification included many complications that could occur in elderly patients, but he couldn't exactly say there was nothing to worry about.
"How about this: I want a bedside ultrasound to take a look. If there is a possibility of Perforation in the appendix, we will go into the operating room immediately. If it perforates, the feces accumulated in the appendix will enter the abdominal cavity, and we must operate. If we wait any longer, it will be life-threatening."
"Doctor, surely not? Didn't you say the possibility wasn't high?"
"The possibility is indeed not high, but there is an old saying that one cannot see into another's heart; instruments can only be used as a reference. You should prepare for emergency surgery first; if there is a problem, we will go to the operating room immediately."
Seeing the Family Member still hesitating, Miao Zong said sternly, "Emergency surgery isn't something we can just do whenever we want. I have to submit a surgical request, and there are specific Anesthesia doctors and Nurses responsible for emergency surgeries. If there are several C-sections scheduled ahead of us, it will be at least the afternoon."
"..."
This statement from Miao Zong seemed to have nothing to do with persuading the Family Member, but upon hearing him say this, the Family Member suddenly felt a vague sense of urgency, as if the opportunity were a rare commodity.
It wasn't something that could be done just because one wanted to; there was a queue.
Miao Zong was very experienced; with one seemingly unrelated sentence, he placed a weight on one side of the scale.
Calling for the Nurse to bring a thermometer, Miao Zong was terrified that the Family Member hadn't heard clearly and would use a temperature gun.
Temperature guns needed calibration; in any case, Miao Zong felt they were not as trustworthy as the old-fashioned mercury thermometers.
"Doctor Xiao Ji, I'm sorry, I have to be busy here for a while." Miao Zong turned back to see Ji Xiang and said very apologetically.
Although it would be fine to take Ji Xiang to his Duty Room to chat for a while now, Miao Zong felt that the patient with Acute Appendicitis was in a very serious condition; he was worried and wanted to see the Ultrasound results.
"Teacher Miao, the patient's condition is a bit off." Ji Xiang didn't mind Miao Zong's apology, but frowned and said, "The tenderness at McBurney's Point is very obvious, but the area is not small, and the referred pain is also very obvious. I just feel it doesn't look much like Appendicitis."
"..." Miao Zong was stunned for a moment.
There was no problem with the patient's diagnosis: it started with migratory pain, then fixed at the McBurney's Point in the lower right abdomen, and there was referred pain.
The location was quite precise; at least it wasn't like many patients who had pain all over their abdomen, making it impossible to determine if it was Acute Appendicitis.
How could Ji Yisheng Xiang say that?!
Although he questioned Ji Xiang in his heart, Miao Zong didn't show it; he just smiled gently and turned to arrange for a bedside color ultrasound.
Just as he was about to make the call, Ji Xiang whispered beside him, "Teacher Miao, can you ask Teacher Kong to come and perform the bedside ultrasound?"
"Teacher Kong? Which Teacher Kong?" Miao Zong was puzzled.
"Director Kong from the Ultrasound Room," Ji Xiang explained.
"!!!"
Miao Zong looked at Ji Xiang with a surprised expression.
Bedside color ultrasound was scheduled by rotation, and the doctors who came were all ordinary junior doctors.
If he were to call Director Kong himself, he would probably be scolded severely, or otherwise, no one would pay him any mind at all.
How could Ji Yisheng Xiang just open his mouth and ask for Director Kong?
Miao Zong was quick-witted; after a moment of thought, he realized that Ji Xiang's relationship with Director Kong might be much closer than his own.
"I can't do it," Miao Zong said truthfully.
"Shall I ask?"
Miao Zong felt Ji Xiang was very strange. It was just an ordinary patient with Acute Appendicitis; why did Ji Xiang keep stepping forward again and again?
And he even wanted to find the Director of the Ultrasound Room to "personally" perform the examination.
If it were an ordinary Intern jumping in to make comments, Miao Zong would have slapped them to the side long ago and ignored them entirely.
But Ji Xiang was different.
Miao Zong had carefully watched every episode of the variety show that had aired, and he had also inquired about many things that had been edited out, giving him a rough understanding.
He saw it as an "ordinary" case of Acute Appendicitis with a risk of Perforation; the patient had a high fever and the Family Member was very scared, but Miao Zong didn't pay much mind to it.
He saw hundreds of similar patients every year; as long as the treatment after Perforation was timely and no sepsis or serious complications occurred, it would be fine.
Even if there were serious complications, the intensive care capabilities of the Affiliated Hospital were very strong; as long as the patient was still breathing when brought in, they could basically come out alive. Miao Zong had this confidence.
Yet Ji Xiang was "making a mountain out of a molehill," believing the patient had a major problem.
Strange.
Miao Zong didn't speak; he watched as Ji Xiang took out his phone, found the number labeled "Director Kong of the Affiliated Hospital Ultrasound Room," and dialed it.
"Teacher Kong, hello, this is Ji Xiang."
"Are you busy? I'm in Gastrointestinal Surgery. There's a patient here who is a bit strange; I don't think it's simple Appendicitis. Could you come and take a look for me?"
"Alright, alright, alright, then I'll have to trouble you."
Although Ji Xiang was on the phone, his tone and body language were very humble; he even held the phone while bowing slightly, as if Kong Zhuren were right in front of him.
Although Ji Xiang acted very humbly, Miao Zong almost burst into tears.
A single phone call could make the boss of the Ultrasound Room come running over pushing a bedside Ultrasound; ordinary Department Heads didn't have this kind of treatment, as at least someone at the Vice President level could achieve this.
Not to mention others, even he himself couldn't do it.
Yet how long had Ji Xiang been here? He was still just a little Intern recording a program.
He could actually summon Kong Zhuren.
Miao Zong had tears welling up in his eyes, feeling inexplicably sad. Look at him, then look at himself, damn it!
"Teacher Miao, Teacher Kong will be here very soon. I feel there are some minor issues with the physical examination and medical history inquiry."
Teacher?
You are the real teacher, Miao Zong muttered to himself.
