Chapter 593: The Meat of the Matter

~7 min read 1,273 words

It's no wonder.

Except for a tiny minority of monstrously gifted individuals, no one can remember every detail in life. This segment just flashed by, and only Bai Chuzhang could spot a few clues and remember them.

The two argued for a few sentences, then simultaneously found common ground—the behind-the-scenes director of this animation must be Ji Xiang!

The perspective of an ordinary person should be on the tragedy of the car accident, and the perspective of a general doctor should be on the critically ill patients in the Emergency Department.

Only Ji Xiang would position the perspective so profoundly on a "discarded" patient.

The animation is beautifully produced; the micro-expressions and small movements never cease to hint to the audience the patient's suffering at this moment.

Watching it the first time, one doesn't feel much, because a multi-vehicle accident is enough to shock the audience. But looking back now, several people have sensed that something is wrong.

For a moment, Bai Chuzhang even wondered if Ji Xiang had truly experienced a horrific car accident.

"Bai Chu, do you all do this normally?" Ding Xiang asked.

"Ding Laoban, let me ask you: when a pile of emergency patients is sent to the Emergency Department and manpower is limited, who do you take blood pressure for first?" Bai Chuzhang asked.

"I don't know." Ding Xiang shook his head.

"I reckon you take the blood pressure of whoever yells the loudest, just like at your Peiyang Barbecue—whoever yells the loudest gets their skewers served first," Liu Zhuren joked.

"How could that be? I always go in order!" Ding Xiang denied Liu Zhuren's claim.

But his expression said it all.

Honest people get bullied everywhere; Ding Laoban didn't intend to bully anyone, he just wanted a little less trouble.

"In the hospital, the doctors and nurses in the Emergency Department must look at those who don't speak first," Bai Chuzhang stared at the screen and said slowly. "People who don't speak might be in Hemorrhagic Shock, or they might be in Traumatic Shock; there are too many possibilities, and they must be treated immediately."

"Then what about those who yell loudly?"

"Patients who still have the strength to yell are fine waiting ten or twenty minutes."

Ding Xiang carefully pondered the various arguments in between; it seemed to make some sense, yet it also seemed like something was wrong.

"Not right!" Bai Chuzhang reached out and pressed on the tablet in front of Ding Xiang.

In the footage, the main perspective was "seeing" the patient's injuries layer by layer through the skin, subcutaneous tissue, muscle, and fascia.

There was no fracture, only soft tissue congestion and edema. The moment Bai Chuzhang reached out and pressed pause, the footage had already advanced into the blood vessels, revealing atherosclerotic plaques within them.

"Atherosclerotic plaques? That's normal."

"Lower limb atherosclerotic plaques superimposed with diabetes, plus trauma. Look here!" Bai Chuzhang reached out and pointed at the television screen.

In the animation, the arterial intima appeared to have a faint tear.

"Thrombosis?"

"Yes. I estimate this patient doesn't look serious, but will end up being the most troublesome one," Bai Chuzhang said cautiously. "Dazhi, let me ask you, what do you do if it's an arterial thrombosis?"

Liu Zhuren thought for a moment and said, "Thrombolysis, what else? The Vascular Department takes over and performs Catheter-directed Thrombolysis."

"What if it's accompanied by lower limb ischemic necrosis? Plus diabetic foot."

"..."

Ding Xiang, the owner of Peiyang Barbecue, didn't understand so many technical terms, but Liu Zhuren did, and his complexion turned extremely ugly.

Rationally analyzing it, the production team wouldn't be "insane" enough to air a follow-up plot where a patient gets an amputation.

After all, it's a variety show, which emphasizes a happy ending.

Everyone comes to watch variety shows; no one is looking to be tortured. If one wants to reflect on life, it's better to go watch those critically acclaimed but unpopular art films.

But a diabetic patient with concurrent lower limb arterial thrombosis—what is to be done next?

"Maybe it's not as serious as you say," Liu Zhuren cracked an ugly smile.

"Heh, not as serious as I say?" Bai Chuzhang said with disdain. "Here, here, and here."

He pointed to three locations in a row.

These three locations all had atherosclerotic plaques, which are also places where thrombosis easily occurs after trauma.

"What are you all talking about?" Ding Xiang was confused and couldn't understand the situation.

"Ding Laoban, come watch the expert the production team just hired show off," Liu Zhuren said with an extremely ugly expression.

"Here, here, and here."

Qin Chu's fingers, as thick as small carrots, pointed at the screen.

"Thrombosis will definitely occur. Boss, what is Lin Jiuze's level of Thrombectomy?"

"Boston Scientific's Thrombectomy Catheter was already FDA-certified in '21, and Lin Jiuze performed some of the surgeries during the phase three and four Clinical trials." Qin Chu's mouth curled upward, becoming increasingly joyful.

"What a coincidence, to run into such a patient." Ling Tian sighed, feeling in his heart that Lin Jiuze was simply a meteor falling from the sky, smashing Ji Xiang into a daze before turning around and leaving.

Even a patient as rare as one with post-traumatic arterial thrombosis appeared on the show.

"I reckon Bo Ke also invested," Qin Chu smiled.

"Heh, the time is too short; I don't think it's very likely," Ling Tian laughed.

As for whether Boston Scientific would invest in the show as an advertisement, it no longer mattered; the point remained on what to do about this "neglected" patient.

After watching the animation again, Ding Laoban of Peiyang Barbecue and Ling Tian in the office of the Interventional Department director at Shanghai Hepatobiliary Hospital both turned their heads to look at the show simultaneously.

In the Review Room, Guangtou Zhuchiren and Professor Shen were chatting, while other guests expressed their surprise with intermittent remarks.

"The commentary is really long."

"The animation before wasn't cut, maintaining the integrity of the plot content, so the expert commentary is all placed at the end," Bai Chuzhang said. "Ding Laoban, do you like this segment?"

"It's alright. I don't dislike it; Professor Shen explains many parts I don't understand," Ding Xiang said.

"There, look," Bai Chuzhang smiled.

The group continued to watch the show. After the previous analysis and judgment, Liu Zhuren knew the subsequent plot would be Lin Jiuze coming on to show off; there was no sense of anticipation at all, and it was insipid.

"I'm leaving first," Liu Zhuren said as he stood up.

"Oh?" Bai Chuzhang glanced at him.

"If it's a thrombectomy, they must be using Bo Ke's High-value Consumables; need you even ask, it must be Lin Jiuze performing the surgery," Liu Zhuren began putting on his clothes. "I get annoyed just looking at Lin Jiuze. Inviting an Expert from Massachusetts General Hospital to participate in a job-seeking show, the production team is truly shameless."

"Are you sure Xiao Ji can't do it?"

"It's a 2021 product; our hospital hasn't even started using it yet. I reckon Xiao Ji hasn't even seen it before, so how could he possibly do it?" Liu Zhuren said with disdain. "Truly fucking shameless!"

"Lin Jiuze might not necessarily be able to perform this surgery," Bai Chuzhang said. "I've learned a bit about Thrombectomy Catheters over these past few years; the requirements are very strict. The slightest carelessness will cause damage to the vascular intima. Arterial plaque is one of the contraindications for a Thrombectomy Catheter."

"Lao Bai, what do you mean?" Liu Zhuren frowned and asked, full of suspicion.
